Starting Counseling at White Oak Counseling & Recovery

Taking the first step toward counseling can feel overwhelming, but our counseling intake process is simple and designed to support you from the very beginning. Whether you are seeking help for yourself, your child, or your family, we will guide you through each step so you know exactly what to expect.

Our counseling intake process in Middleville, Michigan, is designed to make starting therapy simple, clear, and stress-free for new clients. Below is a clear, step-by-step overview of how to become a new client at White Oak Counseling & Recovery.

Step 1: Contact Our Office

To begin services, call our office at 269-205-2402 or submit a request through our appointment form.

During this initial contact, we will gather basic information, including your name, contact details, and insurance details. This helps us match you with the right counselor and prepare for your first appointment.

Step 2: Schedule Your Intake Appointment

Once your information is received, we will schedule your intake appointment. This is your first session, where your counselor will get to know you, understand your concerns, and begin developing a plan for care.

Intake appointments are required before ongoing counseling sessions can begin. We serve clients locally in Middleville, MI, and through telehealth across the entire state of Michigan.

Step 3: Complete Your Intake Forms

After your appointment is scheduled, you will receive an email with a secure link to complete your intake paperwork through our PandaDoc platform.

This allows you to complete and submit all required forms electronically—no printing or scanning needed.

Important Form Deadlines

✔ Intake forms must be completed at least 24 hours before your scheduled appointment.

✔ For Monday appointments, forms must be submitted by Thursday at 5:00 PM.

✔ If forms are not received on time, your appointment will need to be rescheduled.

Completing your forms on time ensures your counselor has the information needed to provide the best possible care during your first visit.

Before your intake appointment, our billing team will review your insurance benefits and contact you regarding any copay, coinsurance, or deductible that may apply.

This helps you understand your financial responsibility ahead of time, so there are no surprises at your visit.
